If you decide that I am a good fit and you’d like to proceed, you will receive access to a client portal to fill out intake paperwork. I’ll gather basic demographic information, medical history, important information about your goals for occupational therapy, and financial and insurance information as needed. Completion of this paperwork will confirm your spot on my schedule.
